UNITED WAY OF HENDERSON COUNTY SUCCESS BY SIX

WOULD LIKE TO INTRODUCE THE AGES AND STAGES QUESTIONAIRE!

 

Ages and Stages is a nationally recognized assessment tool designed to assist you in determining the growth and development of your child between the ages of birth through 5 years.  It's free to the public and very simple to utilize.  The Ages and Stages questionaire will help you to evaluate your child's large and small motor skills, problem solving abilities and social relations.  By clicking on the link below and filling out the requested information, you will automatically be signed up to receive the questionaire by mail which you will be able to fill out in the privacy of your own home, at your own convenience.  Once we receive the completed questionaire, a trained staff member will evaluate your answers and report to you milestones your child has accomplished along with any deficiencies you may be interested in taking further action with along with contact information of experts who can assist you in addressing those issues.  The Ages and Stages Questionaire has been given high rankings by the American Academy of Pediatrics, The American Academy of Neurology, The Child Neurology Society, and First Signs. 

 

Additionally, once signed up, you will receive periodic age appropriate questionaires as your child  continues to grow to ensure that he or she continues to reach developmental milestones that match his or her growth.
